The editorial team strives to use the cover to provide a general rendering of the issue being addressed in the cover story—in this case negative iatrogenic outcomes. Indeed, beyond the extruded paste in the middle of the cover, other iatrogenic-related concerns are evident. We are grateful that some of them have now been identified for our readers. We regret any ill will this may have caused.
