the mandibular anterior gingiva. She reported a swelling on the back of the mandibular
anterior teeth that had increased in size over time. She also reported numbness and
increased mobility of the mandibular anterior teeth that had begun 2 months earlier.
The patient had self-medicated with 500 mg of acetaminophen without significant improvement
of symptoms. Her medical history was significant for leukemia, in remission for 3
years after chemotherapy. Clinical examination revealed localized gingival enlargement,
both facially and lingually, in the region of the mandibular anterior teeth (Figure 1). The enlarged gingiva was bluish red, bosselated, and soft. Periodontal probing
revealed the presence of 4- through 6-mm pockets with significant bleeding. The mandibular
anterior teeth showed grade II and III mobility with tenderness to percussion. The
submandibular lymph nodes were palpable bilaterally. Results of the remainder of the
oral examination were otherwise within normal limits.
